Instructions for Adobe
Acrobat
These forms are in Adobe PDF format and can be filled out and printed
online if you have the Adobe Acrobat Reader which is supplied free by USC at
http://software.usc.edu
You will normally need the Acrobat Software (not just the Reader) to save the
file. However, because of the wide variety of platforms in use at USC, you
should consult with your technical support staff if you have problems saving the
form.
If you are filling out the Statement of Outside Interests,
some additional instructions are in the box below.
When the form comes up on your screen, select the Hand Tool.
When you place the Hand Tool in a field to be filled in with text, it becomes a
bar. Click once and begin entering data.
When you place the Hand Tool in a field that requires a check mark, it becomes a
pointing index finger. Click to insert an check mark in the field.
Note that some of these fields (e.g. Yes/No) only permit one entry so, e.g.,
clicking Yes and then clicking No results in Yes being unchecked and No being
checked.
Once the form is completed, you can print it using either the Acrobat Reader or
your browser's Print command.
Statement of Outside Interests:
Entered data will be italicized. If the field title is short, the
cursor will turn to a
bar to the right of the colon (:). Otherwise,
go to the beginning of the next line. Because the Acrobat
format
does not behave like a word processor, you can overflow some of
these fields, and they will not expand to
accommodate the excess
data. If this happens, please attach additional sheets as the intent
is not to limit data
in any field. |
